You may find it useful and time saving to fill out any required forms needed in advance of the photo shoot. If you are a new customer to a participating Studio, you will need to complete a *registration form. Please note the forms below will require a signature for completion at the time of the shoot. We have also included a standard Model Release form for you to use if you are employing the services of a model/s. This is a basic release and you may want to use your own if this does not cover your particular requirements. Always seek legal advice if you are unsure. You will need a form for each individual model and this includes any members of the public who participate in the shoot.
